RHD's Montgomery County Homeless Services provides a range of services for individuals experiencing homelessness in Montgomery County, PA. It includes Todi's Place of Hope, Genny’s Place, Critical Time Intervention (CTI), Montco Street Outreach, and Homeless Health Initiative.
Todi's Place of Hope and Genny’s Place both offer a safe place to stay and assistance for individuals experiencing homelessness, helping them transition towards independent living. The programs specifically caters to single adults without housing. Clients have access to dedicated caseworkers who help them find permanent housing and apply for necessary benefits. Additionally, we provide practical support such as laundry facilities, transportation, meals, clothing, and essential life skills training.
Critical Time Intervention (CTI) is a blended case management service that provides support, for a duration of nine months, to adults experiencing homelessness in Montgomery County who also have significant mental health needs. Further, CTI aims to understand the impact of trauma on the lives of those we serve. By evaluating program practices and improving our interactions with clients, ultimately the program will have an advanced understanding of the relationships between trauma, substance use, and mental illness.
The Street Outreach team provides support to single adult households actively experiencing homelessness in Montgomery County’s eastern and central regions. The team provides clinical engagement, resource connection, referrals, and person-centered problem solving to stabilize housing situations or find alternative solutions. The team supports individuals in encampments, livening outside, or staying in places unfit for human habitation.
RHD’s Homeless Health Initiative is a collaboration between RHD’s programs providing emergency housing in Philadelphia and Montgomery County. The goal is to help individuals at these programs establish relationships with community-based healthcare services and guide access to the benefits of their health plans, such as vision and dental care. Through this Initiative, they aim to address the social, physical, and behavioral health issues that typically lead to poor health outcomes for this vulnerable population. The Homeless Health Initiative is funded by Keystone First, and it involves close collaboration and information sharing regarding a participant’s medical needs.
